2008-2009 Sports Physicals
WOJFC Physical form required
CHS will be offering sports physicals for the 2008-2009 school year
this May. The cost of the physicals is $15 and it will cover the
student-athlete for the entire 2008-2009 school year. The dates
for the physicals this year are:
Wednesday, May 14 and Thursday, May 15 from 6-8PM
at Orthopedic Associates beside Southview Hospital. (7677 Yankee
Street) Cost of the physicals is $15.

Remember in order for your player to be eligible
to participate in the Wee Elks Football program they must have a
physical completed in 2008. The WOJFC physical form must be completed
and turned in prior to the first practice on July 28th. If your
child isn't eligible for a physical prior to this date, we strongly
encourage you attend these Sports Physicals offered by the High
School.
The Wee Elks WOJFC physical forms will be on hand
for Wee Elks players. This is the only physical form accepted by
the Western Ohio Junior Football Conference (WOJFC). Please contact
Karina Grile at 439-0544, if you have further questions in regards
to the WOJFC physical form.

In order to ensure the Wee Elk Program has sufficient time to order
required equipment for all participants prior to the start of of
practice, a football registration deadline has been imposed for
May 1, 2008. Football registrations may be submitted
after May 1st with a $50.00 late fee, but are subject to return/rejection
based on resource availablity as determined by the Wee Elk Board
of Directors.
